Personal History Statement & Release Forms

Please note:

A completed Personal History Statement will not substitute for a completed employment application. You must complete an online application first, and await instruction from the Police Department's Personnel Bureau.

Click Personal History Statement to complete the form online and print it out. Once the form is completed, signed and notarized, bring it with you.
 
Below is the list of required documents (COPIES) to bring with you:
1.  Birth certificate
2.  College diploma and transcripts, high school diploma and transcripts, or G.E.D.
3.  Social Security Card
4.  DD-214 for each year of military service
5.  Naturalization certificate
6.  Court orders (as appropriate), for matters such as:
A.  Divorce(s)
B.  Marriage certificates(s)
C.  Legal separation(s)
D.  Name change(s)
E.  Bankruptcy(ies)
