The DMS system requires Prince William County staff to verify and validate all contact information entered in the system. This will ensure that the most accurate information is received and staff can provide the best customer service possible using a partnership approach.

Submit the form to County staff in one of the following manners:
- Email the completed form to DMS@pwcgov.org or select the submit button on the electronic form.
- Print and fax to 703-792-5285.
- Mail to DMS Registration, 5 County Complex Court #120, Woodbridge, VA 22192
- Drop off at the Early Assistance Desk located in the Development Services Building.
Staff will review and verify the information. You will receive an email giving you information on your account and instructions on how to log into the new ePortal system!
