Supportive Services Specialists work with family members and older adults to assess, arrange, and evaluate supportive efforts of older adults and their families. Specialists take into account personal factors such as age, income, health, and disabilities to determine the best available options.
A service assessment is required for the following services:

• Ongoing Care Coordination
• Adult Day Healthcare
• Durable medical equipment
