Mental Retardation Case Management services help to assess and link clients and families to needed vocational, recreational and residential programs for individuals with mental retardation. Support and guidance are provided to individuals and their families. Services include:
- information and referral to appropriate services;
- help in accessing vocational, recreational, residential or other services;
- support and guidance to the consumer and his or her family;
- facilitating the delivery of service through an interdisciplinary team process;
- assisting clients in the receipt of Waiver or SPO Medicaid; and
- coordination of services on behalf of clients.
